• Address
• Phone Number
• Your handicap index
• Club where handicap is maintained
• Phone Number of the Club
• Your birthday (month/day - this is optional)
   
• Note: You must have maximum handicap index of 35.8 to join.
• Membership cost is $35 per year (reduced after February 1st).
